(Please submit the following: EIN (Employee Identification Number), Current TB, or Chest X-ray. TB must be current within the last year. Chest x-ray good for 5 years, Background Check Click here to complete the Background Check Requirement, Clear copies of the front and back State driver’s license, social security card, certifications/licensure, and current CPR.) 
STEP 4-  Once you submit all your credentials, you will receive access to complete the application.
