1. Obtain Employee Identification Number (EIN). Click here for instructions. 2. Current TB or Chest X-ray. TB must be current within the last year. Chest x-ray good for 5 years3. Fingerprint Background Check Letter from the Mississippi Department of Health (Not obtained at the local police or sheriff dept). If you do not have an updated background check letter, please contact Precision Healthcare Staffing.4. Clear copies of both front and back of the following documents: a. All professional nursing licenses b. State driver's license - You may take a screenshot and upload it to this application c. Social security card - You may take a screenshot and upload it to this application d. BCLS, ACLS, PALS, NALS, NRP, as applicable5. Current CPR
